style=";font-family:宋體;line-height:**0%;color:rgb(**,**,**);font-size:**px">5.信譽(yù)要求：企業(yè)信用良好，投標(biāo)人（含聯(lián)合體各成員）在各級(jí)建筑市場(chǎng)監(jiān)管平臺(tái)、“信用中國(guó)”各級(jí)網(wǎng)站、國(guó)家企業(yè)信用信息公示系統(tǒng)、各級(jí)行政監(jiān)管部門網(wǎng)站等存在未修復(fù)的行政處罰信息、經(jīng)營(yíng)異常名錄信息、嚴(yán)重違法失信名單(黑名單)信息，不良誠(chéng)信記錄等失信行為的，招標(biāo)人將拒絕其投標(biāo)。企業(yè)信用以《企業(yè)信用承諾函》方式提供（若</span><span style="font-family: 宋體;line-height: **0%;color: rgb(**, **, **);font-size: **px">為聯(lián)合體投標(biāo)，聯(lián)合體成員均需提供），提供虛假承諾的，投標(biāo)將被否決。（承諾函格式自擬）</span></p><p style="margin-left: 0;text-indent: **px">6.投標(biāo)人不得采用掛靠、轉(zhuǎn)包、虛報(bào)業(yè)績(jī)和代為投標(biāo)等形式，一經(jīng)發(fā)現(xiàn)，其中標(biāo)無效，并列入誠(chéng)信黑名單同時(shí)負(fù)責(zé)賠償招標(biāo)人全部損失。</span></p><p style="margin-left: 0;text-indent: style="font-family:宋體">3）聯(lián)合體各方應(yīng)當(dāng)指定牽頭人</span></span><strong><span style="font-family: 宋體;line-height: **0%;color: rgb(**, **, **);font-size: **px">（牽頭人必須為施工單位）</span></strong><span style=";font-family:宋體;line-height:**0%;color:rgb(**,**,**);font-size:**px">，授權(quán)其代表所有聯(lián)合體成員負(fù)責(zé)投標(biāo)和合同實(shí)施階段的主辦、協(xié)調(diào)工作。</span></p><p style="margin-left: 0;text-indent: style="font-family:宋體">4）聯(lián)合體各方簽訂共同投標(biāo)協(xié)議后，不得再以自己名義單獨(dú)投標(biāo)，聯(lián)合體各方不得再以自己名義單獨(dú)或參加其他聯(lián)合體在同一標(biāo)段中投標(biāo)。</span></span></p><p style="margin-left: 0;text-indent:
