簡要規(guī)則描述：本次維修工程項目范圍為：本次大修工程項目范圍：肝腸門診預(yù)防保健樓、檢驗科、北側(cè)垃圾房、醫(yī)療廢棄物倉庫、消防泵房、衛(wèi)生間等6幢房屋與中心主樓四樓。主要維修內(nèi)容：房屋結(jié)構(gòu)加固、消防設(shè)施改造、信息機房改造等。本工程包工、包料、包工期，包質(zhì)量、包安全文明施工、包驗收。具體技術(shù)要求詳見圖紙和工程量清單。
Brief Specification Description:The scope of this maintenance project is: The scope of this major overhaul project includes 6 buildings including the Liver and Intestine Outpatient Preventive Health Building, Laboratory Department, North Side Garbage Room, Medical Waste Warehouse, Fire Pump Room, and Toilet, as well as the fourth floor of the main center building. Main maintenance contents: reinforcement of building structures, renovation of fire-fighting facilities, renovation of information rooms, etc. This project includes labor, materials, construction period, quality, safe and civilized construction, and acceptance. 4、施工資質(zhì)要求：具備國家住建部頒發(fā)的建筑工程施工總承包三級以上（含三級）資質(zhì)及建筑裝修裝飾工程專業(yè)承包二級以上（含二級）資質(zhì)，且具備有效的安全生產(chǎn)許可證；
5、項目負責人資質(zhì)要求：擬派的項目負責人具有建筑工程二級或以上注冊建造師執(zhí)業(yè)資格，并具備有效的安全生產(chǎn)考核證書（B證），且未在其他在建項目擔任項目負責人（注冊建造師基本情況及是否有本市的在建項目以供應(yīng)商提供的開標前兩個工作日上海市建設(shè)市場管理信息平臺采集的數(shù)據(jù)內(nèi)容形成的《項目負責人基本情況表》為準, 項目負責人在履行合同過程中發(fā)生變更的，如變更后時間未滿半年，不得參與本標段投標。）；
6、法人的分支機構(gòu)以自己的名義參與競爭時，應(yīng)提供依法登記的相關(guān)證明材料和由法人出具的對該參與競爭活動承擔全部直接責任的承諾書；
7、法定代表人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項下的采購活動；
8、本項目不接受聯(lián)合體響應(yīng)。
