(c)本項目的特定資格要求：1）具備市政公用工程施工總承包三級及以上資質，并具備有效的安全生產(chǎn)許可證；2）擬派項目經(jīng)理具有市政公用工程專業(yè)二級及其以上注冊建造師執(zhí)業(yè)資格，具備有效的安全生產(chǎn)考核合格證書；3）單位負責人為同一人或者存在直接控股、管理關系的不同供應商，不得參加同一合同項下的政府采購活動；為本項目提供整體設計、規(guī)范編制或者項目管理、監(jiān)理、檢測等服務的供應商，不得參加本項目的采購活動；4）法人依法設立的分支機構以自己的名義參與投標時，應提供依法登記的相關證明材料和由法人出具的授權其分支機構在其經(jīng)營范圍內(nèi)參加政府采購活動并承擔全部民事責任的書面聲明。法人與其分支機構不得同時參與同一項目的采購活動；5）本項目專門面向中型、小型、微型企業(yè)采購；6）本項目不接受聯(lián)合體形式。
