(c)本項目的特定資格要求：3、受委托從事土壤污染狀況調(diào)查、風(fēng)險評估、風(fēng)險管控和修復(fù)、效果評估、后期管理等活動的從業(yè)單位，應(yīng)當(dāng)具備相應(yīng)的專業(yè)能力，配備足夠的管理、技術(shù)和操作人員，環(huán)境或地質(zhì)等相關(guān)專業(yè)高級職稱技術(shù)人員不少于5名，中級職稱技術(shù)人員不少于**名，具有獨立承擔(dān)國內(nèi)同類項目的業(yè)績。從事上述活動的項目負(fù)責(zé)人、報告審核人應(yīng)具有環(huán)境或地質(zhì)等相關(guān)專業(yè)高級技術(shù)職稱；4、投標(biāo)人自身或者委托的分析檢測單位須具備CMA和CNAS證書。投標(biāo)人委托分析檢測單位的需提供檢測單位的CMA和CNAS證書及委托協(xié)議(投標(biāo)人本身自有CMA和 CNAS 證書的，則不需要提供委托協(xié)議),且按規(guī)定向上海市生態(tài)環(huán)境局進行社會化服務(wù)機構(gòu)備案。5、須系我國境內(nèi)依法設(shè)立的法人或非法人組織（本項目不接受分公司以自己名義參加采購活動）；6、本項目不接受組成聯(lián)合體；7、法定代表人或企業(yè)負(fù)責(zé)人為同一人或者存在控股、管理關(guān)系的不同供應(yīng)商，不得參加同一標(biāo)段磋商
