4、施工資質(zhì)要求：具備國家住建部頒發(fā)的建筑工程施工總承包三級以上（含三級）資質(zhì)及建筑裝修裝飾工程專業(yè)承包二級以上（含二級）資質(zhì)，且具備有效的安全生產(chǎn)許可證；
5、項目負責人資質(zhì)要求：擬派的項目負責人具有建筑工程二級或以上注冊建造師執(zhí)業(yè)資格，并具備有效的安全生產(chǎn)考核證書（B證），且未在其他在建項目擔任項目負責人（注冊建造師基本情況及是否有本市的在建項目以供應商提供的開標前兩個工作日上海市建設市場管理信息平臺采集的數(shù)據(jù)內(nèi)容形成的《項目負責人基本情況表》為準, 項目負責人在履行合同過程中發(fā)生變更的，如變更后時間未滿半年，不得參與本標段投標。）；
6、法人的分支機構以自己的名義參與競爭時，應提供依法登記的相關證明材料和由法人出具的對該參與競爭活動承擔全部直接責任的承諾書；
7、法定代表人為同一人或者存在直接控股、管理關系的不同供應商，不得參加同一合同項下的采購活動；
8、本項目不接受聯(lián)合體響應。
