5、項目負(fù)責(zé)人資質(zhì)要求：擬派的項目負(fù)責(zé)人具有建筑工程二級或以上注冊建造師執(zhí)業(yè)資格，并具備有效的安全生產(chǎn)考核證書（B證），且未在其他在建項目擔(dān)任項目負(fù)責(zé)人（注冊建造師基本情況及是否有本市的在建項目以供應(yīng)商提供的開標(biāo)前兩個工作日上海市建設(shè)市場管理信息平臺采集的數(shù)據(jù)內(nèi)容形成的《項目負(fù)責(zé)人基本情況表》為準(zhǔn), 項目負(fù)責(zé)人在履行合同過程中發(fā)生變更的，如變更后時間未滿半年，不得參與本標(biāo)段投標(biāo)。）；
6、法人的分支機構(gòu)以自己的名義參與競爭時，應(yīng)提供依法登記的相關(guān)證明材料和由法人出具的對該參與競爭活動承擔(dān)全部直接責(zé)任的承諾書；
