資格要求：1.如果投標(biāo)人為中華人民共和國(guó)境內(nèi)注冊(cè)公司，需提供有效的營(yíng)業(yè)執(zhí)照（具備獨(dú)立法人資格）、一般納稅人證明/稅務(wù)登記證、組織機(jī)構(gòu)代碼證復(fù)印件或證照合一營(yíng)業(yè)執(zhí)照，投標(biāo)時(shí)提供原件掃描件（原件備查）。如果投標(biāo)人為分公司的，應(yīng)具有有效的證照合一的營(yíng)業(yè)執(zhí)照和總公司合法授權(quán)書?？偣九c分支機(jī)構(gòu)只可一家參與投標(biāo)，同時(shí)參與投標(biāo)視為投標(biāo)無(wú)效。如果投標(biāo)人為事業(yè)單位的，投標(biāo)人應(yīng)經(jīng)全國(guó)各級(jí)事業(yè)單位登記管理機(jī)關(guān)核準(zhǔn)登記或者備案，具有合法有效的事業(yè)單位法人證書。如出現(xiàn)單位負(fù)責(zé)人為同一人或者存在控股、管理關(guān)系的情況，相關(guān)投標(biāo)均無(wú)效，招標(biāo)人隨時(shí)有權(quán)作出拒絕投標(biāo)、取消投標(biāo)資格、取消授標(biāo)、不簽訂合同或在合同簽訂后終止合同等決定，相關(guān)投標(biāo)人須無(wú)條件接受，且由此造成的任何損失由投標(biāo)人承擔(dān)。如果投標(biāo)人為境外注冊(cè)公司，需提供有效的公司登記注冊(cè)證明。2.投標(biāo)人應(yīng)為本次投標(biāo)產(chǎn)品的制造商。本次招標(biāo)不接受代理商、貿(mào)易商、貼牌商、代加工工廠參與投標(biāo)。本次接受所投貨物的制造商全資的銷售公司或控股的銷售公司，以及接受所投貨物制造商的母公司參與投標(biāo)，此類公司視同為制造商，投標(biāo)時(shí)需提供相應(yīng)的證明文件。同一制造商僅允許一家投標(biāo)人參與投標(biāo)，否則相關(guān)的所有投標(biāo)將被否決。3.投標(biāo)人所投產(chǎn)品的制造商須具備有效的GB/T****1（ISO****）質(zhì)量體系認(rèn)證證書或API Q1證書，并可在中國(guó)國(guó)家認(rèn)證認(rèn)可監(jiān)督管理委員會(huì)網(wǎng)站(http://www.cnca.gov.cn/)核實(shí)。如所投貨物的制造商提供的質(zhì)量管理體系認(rèn)證證書是國(guó)外第三方認(rèn)證機(jī)構(gòu)頒發(fā)的，則網(wǎng)站查詢不適用。Qualification Requirements:1.If the bidder is a registered company in the People"s Republic of China, it is necessary to provide a valid business license (with independent legal personality), general taxpayer certificate/tax registration certificate, copy of organization code certificate or three-in-one business license, and provide a scanned copy of the original (original for reference) when bidding. Checking is not applicable to certificates issued by foreign third-party certificate authority.業(yè)績(jī)要求：（開(kāi)標(biāo)時(shí)需進(jìn)行信息公開(kāi)）1.****年1月1日至投標(biāo)截止日（以合同或訂單簽署時(shí)間為準(zhǔn)），投標(biāo)人所投產(chǎn)品的制造商應(yīng)具有1個(gè)合同的不小于**寸的機(jī)械堵漏管卡產(chǎn)品的銷售業(yè)績(jī)。2.投標(biāo)人須按規(guī)定格式提交業(yè)績(jī)表，并提交相關(guān)業(yè)績(jī)證明文件。業(yè)績(jī)證明文件包括但不限于：1）銷售合同掃描件和2）到貨驗(yàn)收材料。投標(biāo)人所提交的業(yè)績(jī)證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁(yè)（應(yīng)有雙方簽署或蓋章）、制造商名稱、貨物名稱、尺寸及到貨驗(yàn)收材料（至少包含：貨物驗(yàn)收單或結(jié)算清單或?qū)?yīng)的結(jié)算發(fā)票（如有清單須提供，須體現(xiàn)發(fā)票號(hào)；發(fā)票需包含合同號(hào)或訂單號(hào)或貨物名稱或買方賣方信息，且與合同或訂單一致）或買方（或最終用戶）接收證明或調(diào)試驗(yàn)收證明或提單（含箱單和發(fā)票）或報(bào)關(guān)單或買方（或最終用戶）簽署的SAT（現(xiàn)場(chǎng)接收試驗(yàn)）報(bào)告。若業(yè)績(jī)?yōu)橛唵?，需提供?duì)應(yīng)的協(xié)議掃描件、訂單掃描件及訂單相應(yīng)的到貨驗(yàn)收材料。貨物驗(yàn)收單或結(jié)算清單至少應(yīng)有合同甲方簽字或蓋章。3.未提交業(yè)績(jī)證明文件，或所提供的業(yè)績(jī)證明文件無(wú)法體現(xiàn)滿足上述業(yè)績(jī)要求的，均視為無(wú)效業(yè)績(jī)。1.
