(5)有依法繳納稅收和社會保障資金的良好記錄；
(6)供應商在近三年內(nèi)無違法犯罪記錄；
(7)未被列入“信用中國”網(wǎng)站(www.creditchina.gov.cn)失信被執(zhí)行人名單、重大稅收違法案件當事人名單和中國政府采購網(wǎng)(www.ccgp.gov.cn)政府采購嚴重違法失信行為記錄名單的供應商；
(8)法人依法設立的分支機構以自己的名義參與磋商時，應提供依法登記的相關證明材料和由法人出具的授權其分支機構在其經(jīng)營范圍內(nèi)參加政府采購活動并承擔全部民事責任的書面聲明。法人與其分支機構不得同時參與同一項目的采購活動；
