Brief specification description or basic overview of the project:According to the requirements of the purchaser, provide medical examination services for the disabled persons with household registration in Pudong New Area who hold the "People's Republic of China Disability Certificate" issued by Shanghai and meet the requirements for medical examination, and complete the health examination of 1,**6 disabled persons in 3 towns including Dongming Road Street. 3、須具有衛(wèi)生行政部門核發(fā)的《醫(yī)療機(jī)構(gòu)執(zhí)業(yè)許可證》；
4、單位負(fù)責(zé)人為同一人或者存在直接控股、管理關(guān)系的不同供應(yīng)商，不得參加同一合同項(xiàng)下的政府采購活動(dòng)；為采購項(xiàng)目提供整體設(shè)計(jì)、規(guī)范編制或者項(xiàng)目管理、監(jiān)理、檢測等服務(wù)的供應(yīng)商，不得再參加該采購項(xiàng)目的其他采購活動(dòng)；
5、本次招標(biāo)不接受聯(lián)合體投標(biāo)，不允許合同轉(zhuǎn)讓與分包。
(c)Specific qualification requirements for this program:Have the ability to independently assume civil liability and the corresponding ability to perform the contract; 2. This project is not specifically for small and medium-sized enterprises; 3. It must have a "Medical Institution Practice License" issued by the health administrative department; 4. The person in charge of the unit is the same person or different suppliers with direct holding and management relationships shall not participate in government procurement activities under the same contract; suppliers who provide overall design, specification preparation or project management, supervision, testing and other services for the procurement project shall not participate in other procurement activities of the procurement project; 5. This invite tenders does not accept consortium bidding, and does not allow contract transfer and subcontracting.
