(c)本項(xiàng)目的特定資格要求：1、****年1月1日至今未被“信用中國(guó)”（www.creditchina.gov.cn）、中國(guó)政府采購(gòu)網(wǎng)（www.ccgp.gov.cn）列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單；
2、須系我國(guó)境內(nèi)依法設(shè)立的法人或非法人組織（本項(xiàng)目不接受分公司以自己名義參加采購(gòu)活動(dòng)）；
3、供應(yīng)商應(yīng)同時(shí)具備以下施工資質(zhì)：1）中華人民共和國(guó)住房和城鄉(xiāng)建設(shè)部頒發(fā)的施工總承包建筑工程三級(jí)或以上資質(zhì)；2）中華人民共和國(guó)住房和城鄉(xiāng)建設(shè)部頒發(fā)的施工專業(yè)承包建筑裝修裝飾工程二級(jí)或以上資質(zhì)。
4、企業(yè)具有有效的安全生產(chǎn)許可證；
5、擬派項(xiàng)目負(fù)責(zé)人具有建筑工程專業(yè)二級(jí)及其以上注冊(cè)建造師執(zhí)業(yè)資格，具備有效的安全生產(chǎn)考核合格證書(shū)，且未擔(dān)任其他在建的建設(shè)工程項(xiàng)目的項(xiàng)目負(fù)責(zé)人（（以響應(yīng)文件中提供的加蓋單位公章的《項(xiàng)目負(fù)責(zé)人基本情況表》為準(zhǔn)（通過(guò)上海市住房和城鄉(xiāng)建設(shè)管理委員會(huì)網(wǎng)站zjw.sh.gov.cn 查詢、打印，首次響應(yīng)文件提交截止時(shí)間前7 天內(nèi)打印有效））。
