簡(jiǎn)要規(guī)則描述：為新片區(qū)公交員工提供體檢服務(wù)。承諾實(shí)行嚴(yán)格的醫(yī)療健康保密制度，不對(duì)外采購(gòu)人的商業(yè)信息及采購(gòu)人員工的個(gè)人信息，并做好體檢結(jié)果保密工作；在體檢前提供體檢套餐咨詢和相關(guān)服務(wù)；在體檢中提供專業(yè)、高效的服務(wù)，并為每位體檢對(duì)象提供一份營(yíng)養(yǎng)早點(diǎn)；在體檢后為每位體檢對(duì)象提供詳盡的健康報(bào)告書；檢后咨詢服務(wù)等。（具體詳見第三章項(xiàng)目概況及服務(wù)需求）
Brief Specification Description:Provide medical examination services for public transportation employees in the new district. Promise to implement a strict medical and health confidentiality system, not to disclose the commercial information of the purchaser or the personal information of the purchaser's employees to the outside world, and to ensure the confidentiality of the physical examination results; Provide consultation on medical examination packages and related services before the physical examination; Provide professional and efficient services during physical examinations, and provide each examinee with a nutritious breakfast; Provide detailed health reports for each subject after the physical examination; Consultation services after inspection, etc. (For details, please refer to Chapter 3 Project Overview and Service Requirements)

(b)落實(shí)政府采購(gòu)政策需滿足的資格要求：本次采購(gòu)若符合政府強(qiáng)制采購(gòu)節(jié)能產(chǎn)品、鼓勵(lì)環(huán)保產(chǎn)品、扶持福利企業(yè)、促進(jìn)殘疾人就業(yè)、促進(jìn)中小微企業(yè)等政策，將落實(shí)相關(guān)政策。
(b)Qualification requirements to be met to implement government procurement policies:If this procurement complies with government policies such as mandatory procurement of energy-saving products, encouragement of environmentally friendly products, support for welfare enterprises, promotion of employment for people with disabilities, and promotion of small and medium-sized enterprises, relevant policies will be implemented.
(c)本項(xiàng)目的特定資格要求：3）須系我國(guó)境內(nèi)依法設(shè)立的法人或非法人組織（本項(xiàng)目不接受分公司以自己名義參加采購(gòu)活動(dòng)）；
4）具有衛(wèi)生局審批的《醫(yī)療機(jī)構(gòu)執(zhí)業(yè)許可證》。
5）本項(xiàng)目面向大、中、小、微型企業(yè)，事業(yè)法人、其他組織或自然人采購(gòu)。
6）符合《關(guān)于對(duì)接國(guó)際高標(biāo)準(zhǔn)經(jīng)貿(mào)規(guī)則推進(jìn)試點(diǎn)地區(qū)政府采購(gòu)改革的指導(dǎo)意見》（滬財(cái)采〔****〕**號(hào)）第**條規(guī)定的供應(yīng)商，不得參加本項(xiàng)目的采購(gòu)活動(dòng)。
7）本次采購(gòu)不接受聯(lián)合體響應(yīng)，不允許合同轉(zhuǎn)讓與分包。
(c)Specific qualification requirements for this program:3) It must be a legal entity or an unincorporated organization established within the territory of China in accordance with the law (this project does not accept branch companies participating in procurement activities in their own name); 4) Having a Medical Institution Practice License approved by the Health Bureau. 5) This project is aimed at large, medium, small, and micro enterprises, public institutions, other organizations, or natural persons for procurement. 6) Suppliers who comply with Article ** of the Guiding Opinions on Promoting Government Procurement Reform in Pilot Areas in Connection with International High Standard Economic and Trade Rules (Hu Cai Cai Cai [****] No. **) are not allowed to participate in the procurement activities of this project. 7) This procurement does not accept consortium responses and does not allow contract transfer or subcontracting.
