(6)供應(yīng)商在近三年內(nèi)無違法犯罪記錄；
(7)未被列入“信用中國”網(wǎng)站(www.creditchina.gov.cn)失信被執(zhí)行人名單、重大稅收違法案件當事人名單和中國政府采購網(wǎng)(www.ccgp.gov.cn)政府采購嚴重違法失信行為記錄名單的供應(yīng)商；
(8)法人依法設(shè)立的分支機構(gòu)以自己的名義參與磋商時，應(yīng)提供依法登記的相關(guān)證明材料和由法人出具的授權(quán)其分支機構(gòu)在其經(jīng)營范圍內(nèi)參加政府采購活動并承擔全部民事責任的書面聲明。法人與其分支機構(gòu)不得同時參與同一項目的采購活動；
