(c)本項目的特定資格要求：1、須系我國境內(nèi)依法設(shè)立的法人或非法人組織（本項目不接受分公司以自己名義參加采購活動）；2、本項目不接受組成聯(lián)合體；3、法定代表人或企業(yè)負責(zé)人為同一人或者存在控股、管理關(guān)系的不同供應(yīng)商，不得參加同一標段磋商；4、須具有衛(wèi)生行政部門核發(fā)的《醫(yī)療機構(gòu)執(zhí)業(yè)許可證》
